Overview

This document is a Building Sewer Trap and Backflow Prevention Device Acknowledgment and Waiver issued by the Ebensburg Municipal Authority. It informs property owners that while sewer traps and backflow prevention devices are generally required, the Authority is currently not requiring their installation in the main lateral serving a property. However, it strongly recommends installing them to prevent odors, sewer gases, and sewage backups.

Required Information

To complete this waiver, the following must be filled out:

  • Property Address: The full address of the property subject to this waiver (including city, state, and ZIP code).
  • Date: The date the form is signed.
  • Property Owner(s) Signature: Signature of the owner(s) or authorized representative.
  • Printed Name: Printed name of the signatory.
  • Witness Signatures: Two witness signatures and printed names are required.

Purpose

This form serves as legal acknowledgment that the property owner has been informed of the risks associated with not installing a sewer trap and/or backflow prevention device. By signing, the owner confirms they are aware of potential issues such as odors, sewer gases, or sewage entering their building and voluntarily waive the requirement for these installations.
